The office is open from 9 am to 5 pm, Monday through Friday. A message can be left after hours.
Our nurse coordinators are involved in many aspects of your care, including scheduling of appointments and hospitalizations as well as seeking insurance approval for transplant procedures. When scheduling an initial appointment, the following information will be needed:
- Name
- Address
- Phone, including cell phone or work numbers
- Date of birth
- Diagnosis
- Insurance information
- Referring physician (or current hematologist/oncologist)
Before your first appointment, please arrange for the following to be sent to our physicians:
- Physician notes (including discharge summaries & history and physicals)
- Treatment records
- All pathology slides and reports (including cytogenetic reports)
- Radiology, MRI, and CT scan reports
- Radiation therapy reports
